La enfermedad pulmonar obstructiva crónica, o EPOC, como será llamada en esta guía, es una enfermedad compleja, prevenible y tratable.

La característica más importante de la EPOC es que durante la respiración, al sacar el aire, este no sale completamente y cada vez se va haciendo más difícil expulsar el aire de los pulmones.

Al mismo tiempo, la EPOC se caracteriza por una inflamación de las vías respiratorias; así pues, las vías por donde pasa el aire se cierran un poco y hacen más difícil expulsarlo; además, se inflaman. Esta combinación de obstrucción e inflamación es lo que produce los síntomas de la enfermedad.

Hay factores ambientales (externos) que interactúan para que algunas personas genéticamente susceptibles (internos) desencadenen la destrucción de las células Beta; estas células son las productoras de la insulina en el páncreas. Algunos de los factores que se han identificado son:

• Familiares de primer grado con diabetes mellitus tipo 1 (DM1).

• Asociación con una enfermedad causada por el sistema inmunitario, que ataca las células del propio organismo. En este caso, el sistema inmunitario se convierte en el agresor y ataca partes del cuerpo.

• Grupos de edades en los cuales se presenta con mayor frecuencia: de los 4 a los 6 años y de los 10 a los 14 años.

Diagnostico

Se puede diagnosticar por la presencia de uno o varios síntomas de diabetes y mediante análisis de la sangre (exámenes de laboratorio). Cualquiera de los siguientes exámenes en sangre (los cuales son solicitados por su médico) hace el diagnóstico de diabetes mellitus:

• Hemoglobina glucosilada ≥ 6,5 %.

• Glucemia plasmática en ayunas 1 ≥ 126 mg/dl.

• Glucemia plasmática dos horas después del test de tolerancia oral a la glucosa (con 75 gramos de glucosa) ≥ 200 mg/dl.

• Glucemia plasmática ≥ 200 mg/dl en pacientes con síntomas clásicos de hiperglucemia o crisis de hiperglucemia.
